The long story short is the Remicade was not working as planned, and I was then started on Humira for my ulcerative colitis.&lt;/p&gt;&lt;p&gt;I can remember the first day I started Humira clearly. I made my way up to my GI doctor's office to meet with his nurse, a woman who at this point was one of my closest friends. (for those of us who have had been given the present of severe Ulcerative Colitis, it is truly amazing how close you can become with your nurses/doctors/even receptionists since they are the first person you see when you arrive at the doctors.&lt;/p&gt;&lt;p&gt;How many people on this planet really don't mind having shots? I feel like I am a macho person sometimes. Fell down playing ice hockey when I was 12 or thirteen and slammed my face against the ice, blood was flying out of my mouth since my teeth cut some things up along with the impact blowing a hole in my lower lip. But needles...for some reason never got used to them. All along, my doctor was telling me how it was no big deal. Well, my doctor was right. Needles like the Humira needle are no big deal. It is so small, I had to blow the picture up to the left just so you could see the thing. Once I arrived, I had my actual prescribed Humira in my hands.&lt;/p&gt;&lt;p&gt;So, on my first day to start Humira, I arrived with my 4 Humira pens and the 4 yellow pages of side effect information that is included. When it came time to do the actual shots, this is when I really started to stall with the nurse. You could ask her, but I believe we may have counted down doing the "ten,nine, eight, seven, wait wait wait wait...." anyways, I was there for about 45 minutes before I had all 4 shots of Humira in me. I was hoping that, when I was supposed to take the medications next(in two weeks) that I would get it going much faster. (That never really was the case, but I did start taking them at home which must have made the physicians assistant happy!)&lt;/p&gt;&lt;p&gt;So that is my story of the first encounter with treating my ulcerative colitis via Humira.&lt;/p&gt;&lt;p&gt;Did Humira Treat My Ulcerative Colitis?&lt;br /&gt;Great great question. One that is a topic of debate among the medical world. Going into taking my Humira I was really severe with Ulcerative Colitis. Has anyone ever been prescribed Humira for Chron's or UC when they were just dealing with a "mild" case of their respective disease? I guess I just assume they save the Humira for when people are not responding to any of the "less dangerous" medications. Well, the fact of the matter is that I was very severe. In looking back on the situation, I would probably only take Humira again if I had tried all the other medications available on this planet including crack cocaine, PCP, opium etc... and going to the bathroom 150 times/day. But that is just my personal decision on when to try Humira again.&lt;/p&gt;&lt;p&gt;Within Two or Three Weeks of Starting Humira:&lt;br /&gt;This was probably about the time where things started getting a bit weird. Someday if the Ulcerative Colitis community thinks it would be valuable, I will pull up my medical records and transcribe them on this website, but for now, I think it was about two or three weeks after starting Humira that some side effects start to appear. It all began as some aches and pains in my upper back/shoulder area. It is the type of thing that anyone can easily pawn off as "oh, I must have slept wrong" or "maybe I pulled a muscle yesterday..."&lt;/p&gt;&lt;p&gt;That is exactly what I did for several days if not a week or more. But as all good things must come to an end, I eventually said no way jose, and dragged myself back the gastroenterology department of the Palo Alto Medical Foundation. My favorite gastro doctor was on vacation while this was happening, so I began working with another excellent doctor until the return of my main one. At this point, I had been under the influence of Humira for maybe about 3 or 4 weeks and I simply could not move. Again I don't remember how bad the skin problems were at this point, but my joints were all messed up. It felt like they emptied out oil in my elbows, shoulders, sternum, and some other parts and I simply was all locked up. To give an example, when it was time to sleep at night, my wife had to push a pillow under my head since my arms were useless. I basically would sit down on the side of the bed, and lay back. Wherever I landed was where I laid. It was almost like dropping off a scuba diving boat, just Humira style. Yeah that's right, my upper body was all messed up including my elbow area. From the waist down, things were fine, thankfully. At least I could walk.&lt;/p&gt;&lt;p&gt;Decision to Stop Humira&lt;br /&gt;As I mentioned, all good things come to an end, and I stopped Humira. This was a decision that was made by me, my temporary GI doctor, and the rheumatologist doctor I saw at the Palo Alto Med. One great part about going to the doctor this day was they prescribed me some Vicodin to take care of the pain. One thing is for sure, Vicodin works on me. Humira does as well, just not how I would have liked, but Vicodin definitely works as intended. The vicodin gave temporary relief, which was better than any present or gift I have received. At this time, there was also a decision made for me to go and up my dose of prednisone to 60mg/day which for a 160lb guy like me is a high amount. But, these were bad times, and drastic measures had to be taken.&lt;/p&gt;&lt;p&gt;Even stranger side effects from Humira:&lt;br /&gt;Along with the Ulcerative Colitis, I now had some horrible joint pains in many spots. The consensus as to what I had come down with was some type of Arthritis. This is actually interesting because Humira is normally prescribed to Arthritis patients to help that. In all honesty, I have a very good friend who is on Humira for arthritis and is doing reasonable good. He has not had any fingernails fall off yet.&lt;/p&gt;&lt;p&gt;For me, another little bonus to go along with the ulcerative colits and joint problems was some real strange bumps on my head. They were something like really large pimples, and they were on my scalp under my hair. And man were they itchy sometimes. The doctors all thought this combination was rather strange, as did my family. I can only imagine what it is like to watch someone in your family go through such strange ordeals. Also, for many of us who have UC, it is not something that just happened overnight, there is usually a long history that goes along with it. So to be adding some more fun on the fire, well lets just say if it don't kill you it makes you stronger. I must be pretty darn strong then!&lt;/p&gt;&lt;p&gt;Time to go to Vegas&lt;br /&gt;After leaving the doctor's office this time, I was heading home with my dad, and the plan was to start up the steroids and also some Vicodin and to get ready for azathioprine. At this point, I was very familiar with all the names of the medication, and this was just another situation where I was going to try out another medication since the previous ones were not getting me better. WRONG. For some reason I can't explain, I always thought the word Azathioprine was scary. maybe its the "z" in there. I did pick up that prescription, but this was the first time I decided not to take it. Basically this was the first time I was going against the doctors orders for any of my treatments up to this point. I wanted to wait until my main gastro doctor was back in the office before popping some more pills. That night, I got home and told my wife the deal, and went online to buy a plane ticket via Orbitz.com and flew on down to Las Vegas, Nevada. I stayed at the Monte Carlo and tried to take my mind adrift, which surely happened.&lt;/p&gt;&lt;p&gt;How much did I like my Ulcerative Colitis at this Point?&lt;br /&gt;Most of the doctors I have met with in the past few years have at times asked me this question: "On a scale of 1 to 10 how bad is the pain?" I was asked some variation of this when I went to meet with the doctors from the previous paragraph. I think this is when I started responding with this type of line: "If I had to choose from UC or this joint problem for the rest of my life, I would chose the ulcerative colitis!" That is what was starting to go on in my head, simply based off how limited you are if you can't move. It would be better to need to go to the bathroom 10-15 times per day, then to not be mobile at all right?&lt;/p&gt;&lt;p&gt;Lets fast forward about 8 months to January 2010.&lt;br /&gt;So, now, it is time to share some photos my current skin problems that started developing back in May of 2009, and its January 2010. Most definitely, the skin problems have gotten much better, but they are still there for sure. I have been prescribed things like Clobetosol which is a very strong steroid cream. UV Rays, other rx creams, and other stuff I don't remember. I really don't think these skin problems are here to stay forever right? But, again, the consensus from the doctors at Stanford Hospital, Mayo Clinic in Scottsdale Arizona, and the Palo Alto Medical Foundation is that my skin problems are a direct side effect from the Humira.&lt;/p&gt;&lt;p&gt;If anyone has similar side effects as this, please contact me, supposedly it is a very small group of people who have this type of reaction to Humira, and I would be truly honored to speak live with someone in the same shoes. Lastly, this particular post is very negative on Humira, or at least that is what I am expecting some people may think. Please realize that my goal is the same as every doctor that is exists on this planet. That is to see as many people as healthy as possible. If you or someone you know is currently taking Humira, and is having great results(much like my good friend) than please congratulate those people. My hope would be that someone considering Humira as a treatment option for ulcerative colitis may actually find a way to treat their UC with a much much smaller chance of having these side effects. Whether to establish a diagnosis on the basis of the presence of microbes or the telltale signs of microbial activity in the urine, to measure the content of specific compounds or metabolites in the urine such as in drug testing applications or any of the numerous other applications, urine analysis is often an important part of health maintenance and courses of therapeutic treatment.&lt;/p&gt;&lt;p&gt;The first step in most urinalysis procedures is to visually inspect the urine, checking for cloudiness in the urine which may indicate an abnormally large amount of proteins or other cellular material in the urine. The color of the urine is also examined, since color can indicate certain conditions; for example, a red or reddish brown cast in a urine sample is often a sign of hemoglobin or myglobin in the urine (though it can also be caused by some drugs, food coloring additives or even eating fresh beets).&lt;/p&gt;&lt;p&gt;Protein screening is another common urine analysis task, particularly in the case of patients providing a cloudy or otherwise turbid urine sample; a protein screen can determine if the patient may be suffering from nephritic syndrome or other kidney ailments. Urine is also commonly screened for glucose content, an excessive amount of which in the urine generally indicated diabetes.&lt;/p&gt;&lt;p&gt;Urinalysis can be used to diagnose some bacterial and viral infections, such as E. coli, which is detectable in urine tests when a positive result is obtained from a nitrite test. Gram-negative rod shaped bacteria (including E. coli) tend to produce these results in when a urine analysis procedure is performed. Urine may also be tested for the presence of bacteria by being cultured and bacterial colonies counted.&lt;/p&gt;&lt;p&gt;The specific gravity of the urine is also generally measured as part of urinalysis procedures - the specific gravity can reveal much about the state of the patient's health, particularly as this relates to their kidneys. This measurement is made by comparing the urine's specific gravity compared to that of plain water.&lt;/p&gt;&lt;p&gt;The pH of the urine is measured using a dipstick and the urine is also checked for the levels of ketones (compounds produced by the breakdown of fats by the digestive system). The concentration of ketones in the urine can indicate diabetes or a insufficient caloric intake, among other conditions. Urine analysis may also include testing for the presence of lysed white blood cells, something which is often symptomatic of a urinary tract infection.&lt;/p&gt;&lt;p&gt;Naturally, a diagnosis is not made on the basis of a urine test alone; a more general examination of the patient is also made before any diagnosis can be definitively established. But what does each one do? What are the major differences? Well, here's scoop:&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Ambulatory:&lt;/strong&gt;&lt;/p&gt;&lt;p&gt;This is a small pump designed to be mobile. This gives patients the freedom to go about their business if they need infusion therapy on a regular basis. And because this type is so convenient and runs on a battery, emergency personnel use them as well. A well known ambulatory pump is an insulin pump.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Syringe:&lt;/strong&gt;&lt;/p&gt;&lt;p&gt;Syringe pumps are designed to deliver small amounts of the particular substance. The medication slowly drips through a syringe. Pain medication, like morphine, is a good example of what syringe pumps are used for.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Enteral:&lt;/strong&gt;&lt;/p&gt;&lt;p&gt;This is basically your basic feeding pump. Adhesives, skin closure tapes, staples and sutures (for example, Covidien or Ethicon sutures) are all ways of repairing lacerations and surgical incisions. Surgical staples and clips are a faster way to close skin wounds, and are more consistent and accurate than hand suturing.&lt;/p&gt;&lt;p&gt;Originally, the first surgical staples were composed of stainless steel. Staple cartridges were reloadable with titanium staples. Today, surgical staplers (like Autosuture or Ethicon staplers) can be made of disposable plastic, or are made of stainless steel and are reusable. The surgical staples are made of either titanium or stainless steel and generally came in disposable cartridges.&lt;/p&gt;&lt;p&gt;As with any wound closure product, sometimes staples produce an inflammatory tissue reaction or an allergic reaction.&lt;/p&gt;&lt;p&gt;Titanium produces less of a reaction than stainless steel. However, patients with nickel allergies should consult with their physicians, as titanium staples contain a certain amount of nickel and could produce a reaction.&lt;/p&gt;&lt;p&gt;Bioabsorbable, synthetic staples based on polyglycolic acid are also on the market. Similar to synthetic absorbable sutures like Vicryl, an Ethicon suture, it is hoped these will cause less tissue reaction. They are also used where permanent staples would not be desirable, for instance, cystotomy repair.&lt;/p&gt;&lt;p&gt;Internal and external wounds can be closed using staples. Normally, a disposable stapler is used to apply skin staples and is removed with a staple remover. Depending on the tissue and the anatomical location to be stapled, the staple line may be straight, circular, or curved. Surgeons prefer using staples in bowel and lung surgery as they are less likely to leak air, blood, or bowel contents. Intestinal staplers are advantageous because they compress wound edges, closing off blood vessels during the stapling process.&lt;/p&gt;&lt;p&gt;Removing staples is done with a staple remover and is a simple procedure. The physician cleans the area, and gently removes each staple by applying pressure to the staple remover so that it bends the staple, straightening out the ends and backing them out of the skin. In the past FIM trials usually enrolled between 20 and 80 healthy volunteers carefully screened for the absence of disease and in part paid for trial participation. However, this is no longer the case. Due to recent concerns over drug safety - for example the Northwick Park experience with TGN1412 causing serious adverse reactions in healthy volunteers - and with the advance of medical science leading to the development of treatments with greater specificity Phase 1 research now increasingly considers patients.&lt;/p&gt;&lt;p&gt;The involvement of patients brings a new dimension to research; patients can conceivability benefit from clinical trials and in considering entering a clinical trial both the patients and the attending doctor must consider the alternatives of treatment. For this latter reason, perhaps, many clinical trials involving patients only consider the terminally ill as candidates for the clinical trials. The perception of benefit also means that patients in Phase 1 clinical trials are not paid. There are also ethical considerations when considering the enrolment of patients. In the conduct of medical care the free will or autonomy of the patient is seen as prime. Failure to respect the wish of patients in deciding their treatment is seen as morally wrong, and in most countries subject to legal sanction. Coupled with the demands of autonomy, and in support of it, are the requirements to fully inform patients about the potential effects of treatment and the alternatives available. Balancing these roles and responsibilities on the medical profession are the requirements that doctors should do no harm and also ensure the just use of resources. These ethical principles are enshrined in the ethical principles of deontology.&lt;/p&gt;&lt;p&gt;In this school of ethical thought it is the intentions not the outcomes that determine morally acceptable behaviour. For example if you have acted in a way that respected patient autonomy, and fully informed them of the outcomes of choices, and the patient has a bad outcome, you have still acted in a morally acceptable manner. In contrast scientific research has a different ethical perspective. It is the magnitude of the outcome that matters in Phase 1 research in order to select a dose for a subsequent trial or the determination of a toxic dose (Shamoo, 2008). Another factor in the conduct of research is that the results should be generalisable to a population of patients in order to extrapolate the meaning of results. In this instance the intention is not to select a treatment for an individual patient. The focus on the outcome of the clinical trial, and the intention to gather generalisable information gives the ethics of clinical research - and Phase 1 clinical research in particular - a so called consequentialist approach. Consequentialist ethics are concerned with the outcome of an action, rather than the intention of the action. Perhaps the best known consquentialist approach is utilitarianism - acting in a way that leads to the benefit for the greatest number. Phase 1 research is often said to be utilitarian in nature as it seeks generalisable knowledge.&lt;/p&gt;&lt;p&gt;The deontological and consequentialist and approaches often lead to different definitions of what is morally correct and acceptable. Consider this. You are standing at a bus stop when a man comes up to you and says he is being chased by an axe murderer. He says he is going to hide in the bushes till the person passes and does so. A few minutes later a man carrying an axe runs up and asks if you have seen anybody, what do you do?&lt;/p&gt;&lt;p&gt;As a consequentialist, you would say you saw nobody as the consequences of your action would lead to murder. As a deontologist in a strict sense you would say he is in the bushes as it is always morally wrong to deceive anybody!&lt;/p&gt;&lt;p&gt;The conflict between the deontological approaches of medical practice and the utilitarian perspective of clinical research in Phase 1 has lead some ethicists to conclude that it is not ethical to enrol patients in Phase 1 studies. The foundation of this claim is that as these are patients their care is paramount, and patients cannot exercise autonomy in their choice as:&lt;/p&gt;&lt;p&gt;1. Phase 1 clinical trials causes more harm to patients than potential benefits.&lt;br /&gt;2. Patients cannot provide a valid informed consent, as they are not consenting for themselves but the collection of generalisable information.&lt;br /&gt;3. Patients whom are terminally ill are vulnerable and so are liable to be manipulated.&lt;/p&gt;&lt;p&gt;Taking these factors in turn:&lt;/p&gt;&lt;p&gt;1. Phase 1 clinical trials causes more harm to patients than potential benefits.&lt;/p&gt;&lt;p&gt;This statement leads to another question - more harm that what or more benefit than what? Both the deontological and consequentialist approaches to medical ethics requires careful consideration of the risks and potential benefits of a medical intervention be it an operation or a clinical trial. In making an assessment of the intervention it is usual to have some form of comparator. In Phase 1 clinical trials three methods have been used. The first is to compare it to the alternative of palliative care - particularly in cancer, but also in inflammatory diseases. The second way of comparing risks and benefits is to look at the approved cancer treatments and draw parallels with the currently licensed medications available from the FDA or the European regulators. The third is to draw parallels with the use of off label medications in cancer (Joffe &amp;amp; Miller, 2006).&lt;/p&gt;&lt;p&gt;The use of a comparison to palliative care is not valid. The patients whom enter a Phase 1 clinical trial have already rejected it as an option at the moment they enrol in a clinical trial. In addition they have not rejected it in the future by enrolling in a trial.&lt;/p&gt;&lt;p&gt;A comparison to existing treatments is also misleading. Many Phase 1 patients have exhausted treatment options so marketed treatments are not a valid comparison. In addition drugs on the marketplace are often registered on the basis of different clinical outcomes than those being measured in clinical trials. Finally as 90% of drugs fail in the development process, the use of a benchmark to existing drugs sets a very high barrier.&lt;/p&gt;&lt;p&gt;Instead perhaps it is more valid to consider the risks and benefits of enrolling in a clinical trial against the use of off label medication by doctors and patients.
